  • Create a list of where your legal documents and insurance policies are kept and who has access to any bank safe deposit boxes. Include user name and password to your computer.
  • Develop a one page list of medications and important medical history
  • Make funeral arrangements. This will ease the burden on your spouse and family.

  • Give to your advocate and review with her/him:
    1. Copy of your Five Wishes Living Will or advance directives
    2. Copy of DNR, if you have one
    3. Copy of health care power of attorney
    4. Copy of health insurance information
    5. Copy of list of emergency contacts
    6. Copy of medical history and medications
